New Patient Information

We carry out a brief medical examination on every new patient over the age of five (5) years. Please bring your completed new patient questionairre and a urine specimen. We are not able to confirm your registration until you have had this examination.

We have the facility to deal with private patients. Please ask the Executive Director for details.
