If you would like to register as a patient at The Limes please pick up a "new registration" pack from our reception team. If you wish to complete this on site, you will need to bring along with you Photo ID preferably your passport or driving licence and proof of address such as a utility bill. Please note: the patient registering must be present with the ID.
Once this has been completed you will be registered onto our computer system and shortly after you will be offered a new patient health assessment. Please bring a specimen of urine along with you for this appointment.
You can complete the registration forms prior to coming into The Limes to save you time. Please click on the Registration Forms tab.
When you register with The Limes you register with the practice not an individual practitioner. However you do as a patient have the right to express a preference to see a GP of your choice.
We will endeavour to do our best to offer you an appointment with the GP of your choice but this may not always be possible. Please take into consideration the following points:-
When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct. Please print off all the relevant registration forms, fill them out and bring them in with you on your first visit to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please print out this questionnaire, fill it in and bring it with you to the practice.
