The Limes Medical Centre is a six partner teaching/training practice with a list size of approximately 19,000 patients.

**NEW** SOCIAL CARE DROP-IN SESSIONS

There will be a social worker on site the first and third Thursday of the month from 9am-5pm to provide information and advice to adults/carers, including:

  • providing details on processes, services they may be able to offer and details of eligibility
  • details on universal services/local services
  • provide details of how to make a referral or support in making a referral (if required)

No appointment necessary.  Please speak to The Limes reception for more details.

TRAVEL CLINIC

Please go to "Clinics & Services" above and then "Travel Advice" to find out about our travel clinic and how we can help you with your vaccinations.  This service is also available privately to non-Limes' patients.

NEW ON-LINE PATIENT SERVICE 

We are pleased to announce that our new on-line system, SystmOnline, is now available to patients.  This means you can use the internet to book appointments with your GP and request repeat prescriptions for any medications you take regularly.

To use this service patients will need to complete the application form, using the link below, and bring it along to one of our surgeries to register.  You will also need to bring some form of photographic ID with you.

Proxy Access

Proxy access allows you to have access to another patient's online services.

An applicant can have proxy access to another patient's online services if;

(a) the patient is aged 16 or over and lacks capacity to make decisions about their healthcare.  In these cases, proxy access must be verified with legal documentation or authorised by a GP;

(b) the patient has given their written consent to allow the applicant proxy access.

OPTING OUT - IMPORTANT INFORMATION

The following information informs you about the scheme that patients can currently choose to opt out of.  Please can you read the following information carefully.

It is important that you are clear of the details of this scheme.

Summary Care Records

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

You can choose to opt out of this scheme.  Please click on the tab below and complete the attached document and return it to The Limes Medical Centre reception where we will code your medical records accordingly.  Please note if you receive care at any other hospitals then you will need to contact them directly.

SHINGLES VACCINATION

A vaccine to prevent shingles, a common, painful skin disease is not available on the NHS to certain people in their 70s.

The shingles vaccine is given as a single injection for anyone aged 70 or 79. Unlike the flu jab, you'll only need to have the vaccination once.

The vaccine is expected to reduce your risk of getting shingles. If you are unlucky enough to go on to have the disease, your symptoms may be milder and the illness shorter.

Shingles can be very painful and uncomfortable. Some people are left with pain lasting for years after the initial rash has healed. And shingles is fatal for around 1 in 1,000 over-70s who develop it.

It's fine to have the shingles vaccine if you've already had shingles. The shingles vaccine works very well in people who have had shingles before and it will boost your immune against further shingles attacks.

Who can have the shingles vaccination?

Shingles vaccination is offered routinely as part of the NHS vaccination programme for people aged 70 or 79. The first people to have the vaccine will be those aged 70 or 79 on 1 September 2013.

If you were aged 70 or 79 on 1 September 2103 but become 71 or 80 before attending the flu vaccination, you will still be able to have the shingles vaccine.

If you are aged 71 to 78 on 1 September 2013, your next opportunity to have the shingles vaccine will be after you have reached the age of 79.
